A considered approach to facial contour and proportion
Hyaluronic acid (HA) dermal filler is a medical treatment that may be used to address selected concerns relating to facial contour, volume distribution and proportion.
At Alluna Medical, treatment planning begins with an individual assessment. We consider facial anatomy, skin and soft-tissue quality, facial movement, previous aesthetic treatment, relevant medical history and the degree of change you are seeking. Treatment is not recommended simply because an area can be injected.
For patients who prefer a subtle change, a plan may focus on proportion, restraint and avoiding unnecessary volume. The decision to treat, the choice of product and the amount used are individual. Results, longevity and suitability vary.
What is Hyaluronic Acid Filler?
Hyaluronic acid (HA) is a substance naturally present in the body, including in the skin and connective tissues. HA dermal fillers are injectable gels used in aesthetic medicine to add volume, support selected contours or modify the appearance of particular facial areas.
Different HA fillers have different physical characteristics. A practitioner may select a product according to the treatment area, the required degree of support or flexibility, tissue characteristics and the treatment objective. Product selection, injection technique and volume should be determined only after medical assessment.
HA filler may create an immediate visible change in volume or contour. However, early swelling, bruising and the normal healing process can temporarily affect the initial appearance. The appropriate timing for review is discussed individually.
What May HA Filler Be Used For?
Following assessment, HA filler may be considered for selected concerns including:

Not every facial concern is best managed with filler. Depending on the assessment, an alternative treatment, surgical opinion, skincare, an energy-based procedure, observation or no treatment may be more suitable.
The Alluna Approach
HA filler treatment at Alluna Medical is doctor-led and based on a whole-face assessment. A line, hollow or feature that concerns a patient may be influenced by adjacent facial structures, volume distribution, skin quality or tissue laxity.
For this reason, we do not approach treatment as a list of isolated areas to fill. We consider whether treatment is appropriate, where it may be placed, whether a staged approach is preferable, and whether further filler would add value.
Our treatment-planning philosophy ties into our Alluna Method, and includes:
Assessing facial proportion rather than treating one feature in isolation
Considering previous filler history before recommending additional treatment
Using conservative volumes where clinically appropriate
Staging treatment when that is safer or more proportionate

The aim is not to create a standardised facial appearance. The intended degree of change is discussed individually during consultation.
Facial Areas Commonly Assessed
Cheeks and mid-face
Changes in mid-face volume and contour can alter the transition between the lower eyelid, cheek, nasolabial fold and lower face. In selected patients, HA filler may be used to adjust cheek contour or provide support in parts of the mid-face.
The term “lifting filler” is sometimes used to describe a visual improvement in contour or support. HA filler does not lift facial tissues in the same way as surgery and is not a substitute for surgical treatment where significant skin or tissue laxity is present.
Tear-trough and under-eye filler
Tear-trough filler may be considered for selected patients with under-eye hollowing or a pronounced shadow between the lower eyelid and cheek.
This is an anatomically complex treatment area and is not suitable for everyone. Prominent eye bags, fluid retention, skin laxity, pigmentation, certain skeletal patterns, previous filler and individual lymphatic factors may affect suitability. In some cases, under-eye filler may not improve the concern and may worsen the appearance of puffiness or swelling.
A recommendation not to inject the tear trough can be an appropriate clinical decision.
Temple filler
Temple hollowing may contribute to a more hollowed or narrowed appearance in some individuals. HA filler may be considered to address selected temple contour concerns.
The temple contains important vascular structures and requires careful assessment, detailed anatomical knowledge and appropriate clinical technique. Treatment may be deferred where it is not appropriate.
Nasolabial folds and marionette lines
Nasolabial folds and lines around the mouth are normal components of facial movement and anatomy. Their appearance may change with facial volume distribution, skin quality, tissue descent and movement.
Rather than automatically injecting directly into a fold, we assess the wider facial context. In some patients, carefully planned treatment may soften the appearance of a selected fold or shadow. Complete removal of normal facial lines is neither realistic nor necessarily desirable.
Chin filler and jawline contouring
Chin and jawline filler may be considered where an individual wishes to discuss lower-face proportion, chin projection, chin length, pre-jowl contour or profile balance.
A chin assessment considers the relationship between the chin, lips, nose, jawline and wider facial proportions. Treatment is planned according to individual anatomy and aesthetic preferences rather than a fixed trend or template.
Non-surgical rhinoplasty
Non-surgical rhinoplasty uses HA filler to modify selected aspects of nasal contour. It may be considered for concerns such as a mild dorsal irregularity, selected asymmetry or a profile-balance concern.
It cannot reduce the size of the nose, address all asymmetries, improve nasal breathing or replace surgical rhinoplasty. The nose is a high-risk filler area because of its vascular anatomy. Non-surgical rhinoplasty is considered only after individual assessment, discussion of alternatives and explicit informed consent.
How Long Does HA Filler Last?
HA filler is generally considered temporary, although the visible effect may be long-lasting. Duration varies according to the product selected, treatment area, volume placed, tissue movement, metabolism, previous treatment and individual response.
Some patients may see a visible effect for several months. In some circumstances, a visible effect may persist for around two years or longer. No specific duration can be guaranteed.
The duration of a visible aesthetic effect does not necessarily correspond to the point at which no filler or treatment-related change may be detectable by imaging. Limited imaging studies and case reports have described MRI findings consistent with HA filler persisting in some facial regions beyond the expected visible duration. Findings vary by treatment area, product, volume, injection plane and individual factors, and should be interpreted cautiously in clinical context.
For this reason, we do not assume that prior filler has fully disappeared simply because a particular period has passed. Previous filler history is part of the assessment before considering additional treatment.
Can HA Filler Be Dissolved?
In some circumstances, HA filler may be reduced with hyaluronidase, an enzyme that breaks down hyaluronic acid. This may be considered when there is unwanted product placement, an unsatisfactory outcome, or as part of the emergency management of certain complications.
Hyaluronidase does not make filler treatment risk-free. The response may vary according to the type and amount of filler, treatment area, tissue plane, time since treatment and individual factors. It may not produce an immediate or complete result in every circumstance, and hyaluronidase itself has potential risks, including allergic reaction.

Filler, MRI Scans and Online Concerns
Questions about filler persistence have become more common following online discussion of MRI imaging. This is a reasonable topic to raise in consultation.
Some limited imaging studies and case reports have reported MRI findings consistent with HA filler in selected facial areas beyond the period during which a visible cosmetic effect might be expected. These findings do not establish that all patients have the same degree of persistence, that an imaging finding necessarily represents a current aesthetic concern, or that additional treatment is always unsuitable. Interpretation requires individual clinical context, including treatment history and examination.
At Alluna Medical, we take a cautious approach to previous filler. We review treatment history where available and do not recommend additional volume solely because time has passed. Depending on the assessment, appropriate next steps may include observation, review after an interval, consideration of further assessment, referral or imaging where clinically appropriate and available, discussion of dissolution, an alternative treatment or no further filler.
Avoiding An Overfilled Appearance
An overfilled or “pillow face” appearance is not an unavoidable consequence of filler, but it can occur when treatment is repeated without adequate reassessment, when excessive volume is used, when filler is placed in an unsuitable area, or when filler is used for a concern better addressed in another way.
The number of syringes used is not a measure of treatment quality. More volume is not inherently better, and a request for additional filler does not always mean that further filler is advisable.
A medically responsible approach includes the ability to pause, reassess, recommend an alternative treatment or advise against additional volume. We consider this part of informed, long-term treatment planning.
Safety, Suitability and Informed Consent
Dermal filler is a medical procedure. Temporary effects may include redness, tenderness, swelling, bruising, discomfort, itching, asymmetry or visible or palpable product.
Potential complications include infection, delayed inflammatory reactions, nodules or lumps, product visibility, changes in skin texture or colour, scarring and reactivation of herpes simplex in predisposed individuals. Although uncommon, vascular occlusion may occur if filler is inadvertently introduced into, or compromises, a blood vessel. This can lead to reduced blood supply, tissue injury, skin necrosis, visual disturbance or loss and, rarely, stroke.
Urgent recognition and management are essential if vascular compromise is suspected. Treatment should only be undertaken in an appropriate clinical setting by a registered medical practitioner with suitable training, assessment processes and arrangements for complication management and follow-up. In Hong Kong, cosmetic procedures involving injections, including dermal fillers, should be performed by registered medical practitioners.
HA filler may be deferred or not recommended where there is active infection or inflammation near the intended treatment area, a relevant allergy history, pregnancy or breastfeeding, a medical condition requiring further consideration, insufficient information about previous treatment, or expectations that cannot reasonably be met.

1. A considered consultation
Your journey begins with a doctor-led consultation. We discuss the concerns you would like to address, the degree of change you are considering, your medical history and any previous aesthetic treatment.
Facial assessment considers more than the area that first draws your attention. We review facial proportion, movement, skin and soft-tissue quality, relevant anatomy, previous filler history and the suitability of any proposed treatment.
A consultation does not mean that filler will be recommended. Depending on the assessment, you may be advised to consider another treatment, take time to decide, follow a staged approach or avoid injectable treatment altogether.
2. An individual treatment plan
If HA filler is considered appropriate, treatment planning is tailored to your anatomy, priorities and the degree of change you prefer.
Some patients prefer a subtle adjustment - for example, restoring selected areas of volume change or refining an aspect of facial contour. Others may seek a more noticeable change, subject to suitability, safety and individual facial proportion.
We do not treat to a fixed number of syringes or a standardised template. The appropriate amount, area and pace of treatment are determined individually. More filler is not inherently better filler.
Some patients prefer the lightest possible refinement; others wish to make a more noticeable change. 3. Treatment and aftercare
If you choose to proceed, the proposed treatment plan, alternatives, expected limitations, potential risks and aftercare are discussed before treatment begins. You will have the opportunity to ask questions and decide whether you wish to proceed.
Temporary swelling, tenderness, redness and bruising can occur. Early changes may not represent the settled appearance. You will receive aftercare guidance, information on what to expect and clear instructions on when to contact Alluna Medical urgently.
4. Review and long-term planning
A review may be recommended where clinically appropriate. Decisions about further treatment are based on your current appearance, treatment history, priorities and whether further filler remains appropriate - not simply on the time elapsed since a previous appointment.
A long-term approach may include pausing treatment, reassessing an area, considering an alternative modality, discussing dissolution where appropriate, or deciding that no further filler is advisable.
Frequently Asked Questions
Is HA filler safe?
HA filler is a medical treatment with potential benefits and risks. Common temporary effects include swelling, bruising and tenderness. Rare but serious complications can include vascular occlusion, tissue injury, scarring, visual loss and stroke. Suitability, technique, informed consent and the ability to recognise and manage complications are important.
How long does HA filler last?
Visible effects vary by product, treatment area, amount used, movement, metabolism and individual response. Some results may be visible for several months; in some circumstances, a visible effect may persist for around two years or longer. HA filler is not permanent, and a specific duration cannot be guaranteed.
Can filler be dissolved?
In some circumstances, HA filler may be reduced using hyaluronidase following medical assessment. The degree and timing of response vary. Hyaluronidase has its own limitations and risks, so it is not a simple or universally predictable reversal process.
Does filler remain in the face longer than expected?
The expected duration of a visible cosmetic effect is not necessarily the same as the duration for which filler or treatment-related changes may be detectable by imaging. Will I look overfilled?
The intended degree of aesthetic change is discussed before treatment. An overfilled appearance can occur when treatment is repeated without reassessment or when excessive volume is used. At Alluna Medical, we take a conservative approach and may advise against further filler when it is not appropriate.
Is tear-trough filler suitable for everyone?
No. Tear-trough treatment is not suitable for all under-eye concerns. Eye bags, fluid retention, skin laxity, pigmentation, anatomical factors and previous filler can affect suitability. Alternative treatment, observation or no treatment may be recommended.
Is non-surgical rhinoplasty safer than surgical rhinoplasty?
They are different procedures and cannot be directly described as safer overall. Non-surgical rhinoplasty does not involve surgery but remains a medical procedure with important risks, including rare severe vascular complications. It cannot reduce nasal size or address functional nasal concerns.
How much filler will I need?
There is no standard number of syringes. Any recommendation depends on anatomy, treatment objective, previous treatment, product selection, safety considerations and whether a staged approach is appropriate. More filler is not necessarily better.
When should I seek urgent help after filler?
Seek urgent medical attention immediately if you experience increasing or severe pain, blanching or whitening of the skin, unusual mottled, dusky or blue-grey skin discolouration, visual symptoms, severe headache, weakness, speech difficulty or other neurological symptoms during or after treatment. Contact Alluna Medical as soon as possible, but do not delay urgent medical assessment while waiting for a response. If symptoms are severe or you cannot contact us promptly, attend the nearest Accident and Emergency Department or call 999 (in Hong Kong).
