Scars are not always obvious on the surface. In some patients, a form of scarring can occur beneath the skin, where fibrous tissue may alter how the skin drapes and moves over the underlying structures.
This deeper process is often described as subdermal fibrosis. In selected cases, it can contribute to tethering, subtle “sinking,” or changes in tissue glide that may be more apparent on examination than in photographs. What is Subdermal Fibrosis (結節)?
Subdermal fibrosis refers to fibrous or scar-like tissue formation beneath the skin, within the supporting layers. In some patients, these fibrotic bands may:
Tether the skin downward.
Restrict normal tissue movement.
Contribute to contour irregularity or a depressed appearance.
Importantly, not all fibrosis is visible to the naked eye. The skin surface may appear reasonably smooth, yet the tissue feels unusually bound down, resistant, or “stuck” when examined, and these changes may only become fully clear during careful palpation or when a cannula is introduced in the correct tissue plane.
Common Causes of Subdermal Fibrosis
Subdermal fibrosis can arise through different pathways. Potential contributors include:
Energy-based devices (EBDs) – such as radiofrequency (e.g. Thermage), radiofrequency microneedling, ultrasound-based lifting (e.g. Ultherapy), and other heat-based tightening treatments, which are designed to deliver energy into the skin to support collagen remodeling. In some patients, higher-intensity or repeated treatments may be associated with prolonged textural change, tightness, or contour irregularity.
Collagen-stimulating injectables or implants – including selected stimulatory fillers or implant materials, which may rarely be associated with nodules, foreign-body granulomas, or fibrotic changes, sometimes months or years after treatment.
Previous surgery or procedural trauma – such as post-surgical depressed scars, revision sites, or areas that have healed with deeper adherence between tissue layers.
Inflammatory acne and chronic inflammation – especially in patients with rolling or tethered acne scars, where fibrotic bands are a defining structural feature.
Traumatic injury – including lacerations, blunt trauma, burns, or wounds that heal with internal adherence.
Repeated procedures in the same area – where multiple energy, injection, thread, or instrumentation sessions may have been layered over time, potentially affecting normal tissue planes.
Not every patient with a history of these treatments will develop subdermal fibrosis, and the presence or absence of fibrosis cannot be inferred from treatment history alone. Proper assessment is required.
Fibrosis after Thermage, Ultherapy, and other energy-based devices
Energy-based devices such as monopolar radiofrequency, radiofrequency microneedling, and micro-focused ultrasound are designed to deliver controlled energy into the skin to support collagen remodeling and perceived firmness in suitable patients.
International reviews and regulatory safety communications have also highlighted that, in some cases, energy-based procedures can be associated with complications such as burns, scarring, fat loss, textural change, induration, nodules, or disfigurement, particularly when technique, settings, or patient factors are suboptimal. These events are considered complications, not expected outcomes in routine practice.
Patients who search online for these issues often use phrases such as “scarring after RF”, “tethering after skin tightening”, “fat loss after RF microneedling”, or “fibrosis after ultrasound lifting”. A medically safer way to frame this is that persistent irregularity, tethering, depression, or tightness after any device-based treatment warrants careful doctor-led review to determine whether subdermal fibrosis, fat atrophy, volume change, or other factors are contributing.

These changes are not always due to fibrosis alone. They may reflect volume, implant behaviour, product placement, tissue reaction, or a combination of factors. Diagnosis is essential before considering any intervention.
How Subdermal Fibrosis May Present
Patients rarely use the word “fibrosis” (“結節”) to describe their concerns. Instead, they might notice:
A tethered or bound-down area under the skin.
A dent, hollow, or “sinking” effect that does not match the surrounding contour.
Tightness or pulling when smiling, speaking, or moving the face.
A patch that feels firmer or less mobile than adjacent tissue.
Textural or contour change that appears more obvious in certain lighting or angles.
Unexpected resistance or “catching” felt during later procedures.
These features can overlap with fat loss, volume deficiency, depressed scarring, implant-related change, or normal anatomical variation. They are signals to seek assessment, not definitive signs of fibrosis.

Why Subcision May Be Relevant

In selected cases of suspected subdermal fibrosis, cannula-based subcision may be considered to gently release fibrotic bands that contribute to tethering or depression. However, subcision is not a universal solution. Some areas may be better managed conservatively, some require staged scar treatment, some may need adjunctive support, and some may not be suitable for treatment at all, depending on the anatomy, prior procedures, tissue quality, and risk profile. The decision to offer or decline subcision is made case by case, after examination and discussion of risks, alternatives, and limitations.
What Treatment May Involve

Important Limitations
Not every post-treatment irregularity represents subdermal fibrosis, and not every suspected fibrosis-related concern should be treated. Some changes may be primarily due to fat loss, age-related volume change, device-related injury, prior surgery, or the natural healing process after inflammation or trauma.

Is every dent or hollow after an energy-based treatment due to fibrosis?
No. A dent or hollow may reflect subdermal fibrosis, but it can also be due to fat loss, normal ageing, volume change, implant behaviour, or the way tissues healed after inflammation or trauma. A consultation is needed to clarify the main contributors before deciding whether treatment is appropriate.
Can subdermal fibrosis always be “reversed”?
Not always. In some patients, selected techniques such as cannula-based subcision may help reduce tethering or improve contour, but improvement is often partial and gradual rather than complete, and there are situations where attempting to “reverse” deep tissue change would not be advisable. We will discuss realistic expectations and when observation may be safer.
