This Personal Information Collection Statement (“Statement”) is provided by BCHK Medical Group Limited trading as Alluna Medical (“Alluna Medical”, “we”, “us”, or “our”) in accordance with the Personal Data (Privacy) Ordinance (Cap. 486) of Hong Kong (“PDPO”).
1. Why we collect your data
We may collect, hold, process, use and disclose your personal data and health-related information for purposes directly related to our services and operations, including:
patient registration, appointment booking, identity verification and administration;
medical assessment, diagnosis, treatment, procedures, prescriptions, follow-up care and continuity of care;
arranging referrals, laboratory tests, imaging, pharmacy services and other ancillary healthcare services;
maintaining medical records and clinic documentation;
billing, payment processing, refunds, debt recovery, insurance pre-authorisation, claims and related administration;
communicating with you about appointments, reminders, test results, aftercare, safety information and other service-related matters by phone, SMS, WhatsApp, email or post;
handling enquiries, feedback, incidents, complaints, quality assurance, risk management and staff training;
complying with legal, regulatory, professional, licensing, infection control, public health, audit and reporting obligations;
protecting your vital interests in urgent circumstances, including contacting your emergency contact where reasonably necessary.
2. What data we may collect
The data we collect may include:
your name, title, sex, date of birth, HKID/passport or other identification details;
contact details, address, emergency contact details and communication preferences;
payment and insurance details;
medical history, medications, allergies, past procedures, examination findings, clinical photographs, laboratory or imaging results, referral letters and other health information relevant to your care;
website, booking and communications data, such as IP address, browser/device information, cookies, enquiry records and call/message logs.
3. Mandatory or voluntary provision
Some personal data is mandatory for us to register you, verify your identity, provide medical care, maintain proper medical records, process payment, or comply with legal and professional requirements. If you do not provide the personal data marked as mandatory, we may be unable to provide or continue services to you. Other requested data is voluntary, but may help us deliver safer or more tailored care.
4. Who your data may be shared with
Your personal data may be transferred or disclosed, where appropriate and on a need-to-know basis, to:
doctors, nurses, allied health professionals, reception, administrative staff and authorised data processors of Alluna Medical;
laboratories, radiology/imaging centres, pharmacies, hospitals, day procedure centres and other healthcare providers involved in your care;
your referring doctor, family doctor or other healthcare professionals involved in your treatment, where relevant;
insurers, medical financing administrators, claims handlers or payment service providers for billing and claims purposes;
IT, cloud, EMR, payment, communications, cybersecurity, document management and other service providers engaged by us under duties of confidentiality and security;
government authorities, regulators, law enforcement agencies, courts, tribunals or professional bodies where disclosure is authorised, required, or permitted by law;
any other person with your prescribed consent or as otherwise permitted under the PDPO.
5. Data security and retention
We take practicable steps to protect personal data against unauthorised or accidental access, processing, erasure, loss or use, including administrative, technical and physical safeguards. Personal data will be retained only for as long as reasonably necessary to fulfil the purposes for which it was collected, or as otherwise required or permitted by law, professional guidance or legitimate operational needs.
6. Access and correction
You have the right to request access to, and correction of, your personal data held by us in accordance with the PDPO. We may charge a reasonable fee for complying with a data access request as permitted by law.

7. Direct marketing
We will not use your personal data for direct marketing unless we have obtained your express and voluntary consent or indication of no objection in the manner required by the PDPO. Your consent is optional, and you may withdraw it at any time by contacting us in writing. Service-related messages about your care, appointments, safety or account are not direct marketing.
8. Clinical photographs and treatment records
If clinical photographs are taken for identification, assessment, treatment planning, monitoring progress, or inclusion in your medical record, they will be handled as part of your confidential medical record. Any separate use beyond your care and internal clinical documentation, such as education, publication or marketing, will require separate consent where applicable.
9. Website and online enquiries

本《收集個人資料聲明》(「本聲明」)由 BCHK Medical Group Limited(以 Alluna Medical 名義經營)(下稱「Alluna Medical」、「本診所」、「我們」)根據香港《個人資料(私隱)條例》(第486章)(「私隱條例」)提供。
1. 收集資料的目的
本診所可為與服務及營運直接有關的目的,收集、持有、處理、使用及披露閣下的個人資料及健康資料,包括:
病人登記、預約、身份核實及行政安排;
醫療評估、診斷、治療、程序、處方、覆診及持續護理;
安排轉介、化驗、影像檢查、藥房服務及其他輔助醫療服務;
保存醫療紀錄及診所文件;
收費、付款處理、退款、追收欠款、保險預先批核、索償及相關行政工作;
透過電話、短訊、WhatsApp、電郵或郵寄,就預約、提示、檢驗結果、術後護理、安全資訊及其他與服務相關事宜與閣下聯絡;
處理查詢、意見、事故、投訴、質素保證、風險管理及員工培訓;
遵守法律、監管、專業、牌照、感染控制、公共衛生、審計及申報責任;
在有需要保障閣下重要利益的緊急情況下,聯絡閣下之緊急聯絡人。
2. 我們可能收集的資料種類
我們可能收集的資料包括:
姓名、稱謂、性別、出生日期、香港身份證/護照或其他身份資料;
聯絡資料、地址、緊急聯絡人資料及通訊偏好;
付款及保險資料;
與閣下護理相關之病歷、現用藥物、過敏史、既往療程、檢查結果、臨床照片、化驗或影像報告、轉介信及其他健康資料;
網站、預約及通訊資料,例如 IP 地址、瀏覽器/裝置資料、cookies、查詢紀錄及來電/訊息紀錄。
3. 必須提供或自願提供
部分個人資料是本診所為替閣下辦理登記、核實身份、提供醫療服務、保存妥善病歷、處理付款或遵守法律及專業要求所必需。如閣下未能提供列為必需的個人資料,本診所可能無法向閣下提供或繼續提供服務。其他資料則屬自願提供,但有助我們提供更安全或更切合個人需要的護理。
4. 資料可轉移予哪些人士
閣下的個人資料可在適當及有需要知道的基礎上,轉移或披露予:
Alluna Medical 的醫生、護士、專職醫療人員、接待及行政人員,以及獲授權資料處理者;
參與閣下護理的化驗所、放射/影像中心、藥房、醫院、日間醫療中心及其他醫療服務提供者;
與閣下治療有關的轉介醫生、家庭醫生或其他醫療專業人員;
保險公司、醫療融資行政機構、理賠處理方或付款服務供應商,以作收費及索償用途;
由本診所聘用並受保密及保安責任約束的資訊科技、雲端、電子病歷、付款、通訊、網絡安全、文件管理及其他服務供應商;
在法律授權、規定或容許下的政府部門、監管機構、執法機關、法院、審裁處或專業團體;
經閣下明確同意的其他人士,或私隱條例另行容許的其他人士。
5. 資料保安及保存
本診所會採取切實可行措施,保障個人資料免遭未獲授權或意外查閱、處理、刪除、遺失或使用,包括行政、技術及實體保安措施。個人資料只會保存至達成收集目的所需的合理期間,或按法律、專業指引及正當營運需要而保存。
6. 查閱及更正資料
根據私隱條例,閣下有權要求查閱及更正本診所持有的閣下個人資料。本診所可按法律容許收取合理查閱資料費用。

7. 直接促銷
除非已按私隱條例規定取得閣下明確及自願的同意或表示不反對,否則本診所不會使用閣下的個人資料作直接促銷用途。閣下可隨時以書面通知我們撤回有關同意。與閣下醫療護理、預約、安全或帳戶有關的服務訊息,並不屬直接促銷。
8. 臨床照片及治療紀錄
如本診所為身份識別、評估、治療計劃、療效監察或保存病歷而拍攝臨床照片,有關照片將作為閣下機密醫療紀錄的一部分處理。如需將該等照片用於超出閣下護理及內部臨床紀錄以外的用途,例如教學、刊物或市場推廣,將按適用情況另行取得獨立同意。
9. 網站及網上查詢
